On or about December 4th, 2023, an employee engaged in maintenance related operations located at 2221 East 4th Street, North Platte, Nebraska 69101, was exposed to caught in hazards resulting in an amputation to his finger while attempting to troubleshoot the Rigid pipe threader.

29 CFR  1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.   On or about December 4th, 2023, employees engaged in maintenance related operations located at 2221 East 4th Street, North Platte, Nebraska 69101, were exposed to amputation hazards in that employee suffered an amputation from troubleshooting a pipe threader.  The employer failed to report the amputation within the required time frame.
