29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:  (a)	On or about February 6, 2024, employees doing roof work on a new build single-family two (2) stories house and were exposed to eye injuries by using nail guns to apply roof shingles to the roof and not have safety glasses.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  On or about February 6, 2024, employees doing roof work on a new build single-family two (2) stories house and were exposed to fall hazards by using extension ladders that did not extend 3-feet above the landing of the roof.   (a)	Werner 28-feet at North end (side) of the house Employees were exposed to 20-feet fall hazard.  (b)	 Louisville 28-feet extension ladder at the South end (front) of the house. No model number and serial number could be recognized.  Employees were exposed to 20-feet fall hazard.  (c)	Werner 24-feet extension ladder at the West end (side) of the house. No model number and serial number could be recognized.  Employees were exposed to 10-feet fall hazard.  (d)	Werner 24-feet extension ladder at the East end (back) of the house. Employees were exposed to 10-feet fall hazard.
