29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems:   a) New single-family residential structure located at 6250 Bennett Parkway, Cumming, GA 30040 (Lot 44 The Park at Bethelview subdivision), Lower Roof: On or about 02/05/2024, the employer exposed employees to fall hazards of approximately 12-feet, to the lower ground level, in that a fall protection system was not provided or utilized while performing roofing operations on a 2/12 pitched roof.

29 CFR 1926.502(a)(2): The employer did not provide and install all fall protection systems required by this subpart for employee(s) and did not comply with all other pertinent requirements of this subpart before employee(s) began the work that necessitates the fall protection.  a) New single-family residential structure located at 6250 Bennett Parkway, Cumming, GA 30040 (Lot 44 The Park at Bethelview subdivision), Roofing area: On or about 02/05/2024, the employer exposed employees to fall hazards of 21-feet, to the lower level, in that fall protection system provided was not used as intended. The fall protection system life-line was tied in a knot around the roof trusses without the use of a metal anchor plate and snap lock connector as provided with the system.

29 CFR 1926.503(a)(2)(ii): The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used:  a) New single-family residential structure located at 6250 Bennett Parkway, Cumming, GA 30040 (Lot 44 The Park at Bethelview subdivision), Roofing area: On or about 2/5/2024, the employer exposed employees to fall hazards of approximately 12-feet, to the lower ground level, in that employees were not trained to erect and inspect fall protection systems before the shift began to eliminate potential falls due to system failure as fall protection systems were not used as intended by the manufacturer.
