29 CFR  1910.215(a)(4):  Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  On or about February 6, 2024, and at times thereto, in the fabrication shop, employees utilized grinders with work rests positioned more than one-eighth inch from the abrasive wheels when performing machining activities:  a.  gray pedestal grinder on the west wall, and b.  Ryobi grinder in the middle of the shop.

29 CFR  1910.303(c)(3)(i):  Conductors were not spliced or joined with splicing devices identified for the use or by brazing, welding, or soldering with a fusible metal or alloy. Soldered splices were not first spliced or joined to be mechanically and electrically secure without solder and then soldered. All splices and joints and the free ends of conductors were not covered with an insulation equivalent to that of the conductors or with an insulating device identified for the purpose:   On or about February 6, 2024, and at times thereto, in the machine shop, employees utilized an industrial pedestal fan with an open, spliced power cord while operating boring mills.
