29 CFR  1910.178(l)(1)(i):  The employer did not ensure each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  (a) On February 6, 2024, and times prior to, the employer did not ensure each employee who operated the Toyota Electric Forklift Truck Order Picker (serial #8BPUE15-05716) completed the required training and evaluation.  (b)  On February 6, 2024, and times prior to, the employer did not ensure each employee who operated the Toyota Electric Forklift Truck (serial #25255) completed the required training and evaluation.
