(a) On or about February 8, 2024, located at 4595 Fenley Way, Brookfield, WI 53005, the employer failed to ensure employees using pneumatic nail guns were wearing appropriate eye protection, exposing employees to eye hazards from flying objects.

29 CFR  1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (a) On or about February 8, 2024, located at 4595 Fenley Way, Brookfield, WI 53005, the employer failed to ensure employees installing shingles on the west side of a two-story residential home were using fall protection, therefore exposing employees to a fall hazard of 16 feet.

(a) On or about February 8, 2024, located at 4595 Fenley Way, Brookfield, WI 53005, the employer did not ensure that employees exposed to potential fall hazards had been trained to recognize and minimize the fall hazards associated with performing roofing work on a two-story residential home with a pitch of approximately 10:12.
