29 CFR 1910.212(a)(1): The employer failed to ensure one or more methods of machine guarding be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  a. On or about February 1, 2024, the employer failed to provide effective guarding to protect employees working at or near the Jet 20" Step Pulley Drill Press Model: JDP-20MF, Serial Number: 21039542. A machine operator was drilling holes into seat blocks while locked in place with the jig, when their gloved left ring finger was amputated after becoming entangled with the rotating drill press bit.

29 CFR 1910.213(p)(2): The employer failed to ensure drum sanding machine(s) have an exhaust hood, or other guard if no exhaust system is required, so arranged as to enclose the revolving drum, except for that portion of the drum above the table, if a table is used, which may be necessary and convenient for the application of the material to be finished.  a. Woodworking Shop - On or about February 8, 2024, the employer failed to provide effective guarding to protect employees working at or near the back assembly station's drum sanding equipment. A machine operator used a Baldor Motor retrofitted with a rotating drum to sand the approximately 12-inch back assembly piece with the operator's hands working within approximately 6 inches of the unguarded rotating sanding drum, thereby exposing the employee's hands and fingers to entanglement and amputation hazards.

And/or the employer did not ensure employees are trained in the requirements of this paragraph on or before May 17, 2017.  a. Vinyl Rolls - On or about February 8, 2024, the employer did not ensure employees that used mobile ladder stands and were exposed to a fall hazard were trained in the requirements of 29 CFR 1910.30, including training on the proper use, care, inspection, and storage requirements of the mobile ladder stands.
