29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  a- Building Exterior:  An employee engaged in pressure plate installation activities from the lift basket of an aerial lift was exposed to fall hazards of approximately 12ft to the asphalt below as a personal fall arrest was worn but not secured so as to prevent falls. Condition noted on or about 02/12/2024.
