29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour:  In the Linemen shop and the electricians work area several Oxygen cylinders and fuel gas cylinders (Acetylene) were in storage and were not separated as required by the regulation.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listed or labeling.    In the breakroom: The Inland Surge Guard with 6 outlets and being used as an extension cord to energize a Panasonic microwave (Serial No. 6A9305 1038) was not listed, labeled or certified for this application.   Electricians work area: The 4-inch square metal electrical box with knockouts and with a duplex receptacle attached to the end of the black flexible cord was not securely affixed to any surface. The assembly was used  as an extension cord to energize portable power tools and was not listed, labeled or certified for this application.

29 CFR 1910.215(b)(9):29 CFR 1910.215(b)(9):  The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch.  Electrician work area: An 8 inch JET Equipment bench grinder (Serial No. 6046740) tongue guards was not adjusted to 1/4 of an inch. The tongue guards opening was about one inch. Employees were exposed to face injuries.
