This condition exposed the employee to an arc flash hazard.

29 CFR 1926.961(c)(2): The employer did not ensure that all switches, disconnectors, jumpers, taps, and other means through which known sources of electric energy may be supplied to the particular lines and equipment to be de-energized were open.  The employer did not render such means inoperable and did not ensure that such means were tagged to indicate that employees were at work.  Data Mine Construction: On or about February 05, 2024, the employer did not ensure the switch supplying energy to a transformer was open, inoperable, and tagged to indicate that employees were at work. This condition exposed employee(s) to an arc-flash hazard.
