29 CFR  1926.100(a):Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  a) Ground level: On 2/13/2024, the employer exposed employees working on lower levels to struck-by hazards, in that head protection was not provided to employees cleaning below roof shingle removal activities located on the roof.

29 CFR  1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a) Roof: On  2/13/2024, the employer exposed employees to struck-by hazards, in that no eye or face protection was not provided while using pneumatic nail guns, hammers, and circular saws.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems:  a) Commercial construction, roof: On 2/13/2024, the employer exposed employees to fall hazards of approximately 11-feet, 2-inches to the lower ground level, in that a fall protection system was not provided to employees engaged in roofing activities including the installation of roofing underlayment and removing shingles from the old roof.
