29 CFR 1910.119(f)(1):The employer did not develop and implement written operating procedures that provided clear instructions for safely conducting activities involved in each covered process: (a)Dynamic Renewables, Inc at 19051 US-81, Lake Norden, SD 57248: On or about and at times prior to February 15, 2024, the employer did not ensure that valves, fans, pumps and other equipment listed on the operational steps of the Upgrade Operations OP-502 Revision 0 were clearly identified to ensure employees performed operational steps utilizing the correct equipment. Examples include but are not limited to: -Section 5.2.3: Ensure open manual gas inflow and gas outflow valves (Page 4 of 16). -Section 5.3.12: At the HMI, set both fan and valve to auto (Page 5 of 16). -Section 10.2.20: Close manual valve directly downstream of the recirculatory pump (Page 14 of 16). -Section 10.2.22: Open the valve behind the pump slowly (Page 14 of 16). -Section 10.2.25: Check, open and manually adjust the hand valve in the control loop for redox, pH and temperature measurements (Page 14 of 16). -Section 10.2.44: Open the ball valve between the bioreactor and sulfur settler (Page 15 of 16). Inadvertent operation of equipment or failure to isolate key equipment can cause or contribute to a release of a highly hazardous chemical, exposing employees to physical and chemicals hazards associated with raw biogas and other process chemicals.
