29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.   On February 16, 2024, and times prior thereto, employees work from a pole scaffold without fall protection systems while installing cornice material on the exterior of a residential home. The employees were exposed to fall hazards of approximately 14 feet to the ground below.

29 CFR 1926.452(a)(4): Diagonal bracing in both directions were not installed across the entire outside face of all double- and single-pole scaffolds.   On February 16, 2024, and times prior thereto, employees working from a pole scaffold without cross bracing installed were exposed to a collapse and falling approximately 14 feet to the ground below.

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used.   On February 16, 2024, and times prior thereto, employees performing cornice work utilized an extension cord with the outer sheathing split exposing internal wiring. The employees were exposed to electrical shock hazards.

The employees were exposed to potential fall hazard of approximately 14 feet to the ground below.
