29 CFR  1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  a)  On or about February 20, 2024, at the worksite 7155 Buckley Road, North Syracuse, NY 13212: Employees were engaged in residential roofing activities on a 7:12 and 9:12 pitch roof.  The employer failed to protect employees from falling by use of guardrail systems, safety net systems, or personal fall arrest system.  Employees were exposed to falls of up to 18 feet.

29 CFR  1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a)  On or about February 20, 2024, at the worksite 7155 Buckley Road, North Syracuse, NY 13212: Employees were engaged in residential roofing activities and accessing the roof with a portable ladder.  The employer failed to ensure the ladder's side rails were extended 3 feet above the access point.
