29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provided for an alternative fall protection measure:   a) Jobsite, 4035/4037 California Avenue, Pittsburgh, PA: On or about February 21, 2024, during roofing activities, the employer did not ensure that employees used appropriate fall protection systems/devices where fall hazards of at least approximately 20 feet existed.

29 CFR 1926.20(b)(2): The employer did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers:    a) Jobsite, 4035/4037 California Avenue, Pittsburgh, PA: On or about February 21, 2024, during construction activities, the employer did not ensure that a system was in place to identify and correct safety and health hazards. Deficiencies noted during the inspection included, but were not limited to, failing to use fall protection where fall hazards of at least 20 feet existed, not ensuring that a safe means of access/egress was provided to/from the roof and failing to use head protection where working below areas where items above could drop or failing to barricade the area off to prevent entry.
