29 CFR  1910.147(c)(2)(ii): The employer's tagout energy control program procedures did not successfully comply with 29 CFR 1910.147(c)(3) and no lockout was provided for employee protection.  (a) On or about February 21, 2024, located at 150 Holy Hill Road, Twin Lakes, WI 53181, the employer did not ensure that the use of tagout energy control program complied with 1910.147(c)(3) in that tags were not required to be affixed to the same location as locks and that all energy sources were rendered isolated and safe prior to entry into the MAAC #1 Thermoform machine, exposing employees to thermal energy hazards with temperatures of approximately 200 degrees Fahrenheit.   (b) On or about February 27, 2024, located at 150 Holy Hill Road, Twin Lakes, WI 53181, the employer did not ensure that the use of tagout energy control program complied with 1910.147(c)(3) in that tags were not required to be affixed to the same location as locks and that energy sources were rendered isolated and safe prior to entry into the MAAC #10 Thermoform machine, exposing employees to struck-by hazards.

(a) On or about February 27, 2024, located at 150 Holy Hill Road, Twin Lakes, WI 53181, the employer did not supply employees with a means of attaching tagout devices that were non-reusable, self-locking, and non-releasable with a minimum unlocking strength of no less than 50 pounds to prevent inadvertent or accidental removal, while performing set-up operations on the MAAC #10 Thermoform Machine.

29 CFR  1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6).  (a) On or about February 16, 2024, located at 150 Holy Hill Road, Twin Lakes, WI 53181, the employer did not ensure the employee implemented energy control procedures prior to entering the point of operation of the MAAC#1 Thermoform machine to perform troubleshooting with the clamps used to secure the mold. The machine was not isolated from all energy sources prior to engaging in these troubleshooting activities, exposing employees to severe burns from the thermal energy source.  (b) On or about February 27, 2024, located at 150 Holy Hill Road, Twin Lakes, WI 53181, the employer did not ensure employees implemented energy control procedures prior to entering the point of operation of the MAAC#10 Thermoform machine to perform set-up operations consisting of but not limited to: mold changing was conducted. The machine was not isolated from all energy sources prior to engaging in these set-up servicing, exposing employees to mechanical energy hazards.
