29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  On or about February 16, 2024, on the southside of a single level residential building, employees of a subcontractor used an extension ladder to access the roof of the building and the ladder's side rails did not extend at least 3 feet above the upper landing surface.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  On or about February 16, 2024, on the roof of a single level residential building, employees of a subcontractor  were not in use of fall protection when performing roofing work activities on a roof that was more than six(6) feet from the ground.  Forrester Brothers Roofing & Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 1543298, citation number 1, item number 1 and was affirmed as a final order on September 14, 2021, with respect to a workplace located at 3421 W Rock Creed Rd Norman, OK 73072.
