29 CFR  1910.134(d)(2)(i): The employer did not provide either a full facepiece pressure demand SCBA certified by NIOSH for a minimum service life of thirty minutes or a combination full facepiece pressure demand supplied-air respirator with auxiliary self-contained air supply for employee use in Immediately Dangerous to Life or Health (IDLH) atmospheres:   On or about January 22, 2024 and times thereafter, employees working at, but not limited to, the Vital Varencia Unit 407, entered compressor unit site(s) with potential IDLH atmospheres without a pressure demand supplied-air respirator or an auxiliary self-contained air supply respirator to perform maintenance.

29 CFR  1910.134(g)(3)(i): One employee or, when needed, more than one employee was not located outside the immediately dangerous to life or health atmosphere to provide emergency rescue:   On or about January 22, 2024 and times thereafter, employees working at, but not limited to, the Vital Varencia Unit 407, were performing maintenance on compressor unit(s) was exposed to 99 parts per million (ppm) of hydrogen sulfide (H2S) as a ceiling concentration, in which the immediately dangerous to life and health (IDLH) level is 100 ppm, and the employer did not locate an employee outside the IDLH atmosphere to provide emergency rescue.
