29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure.  In the tool crib, on the north wall, an extension cord was wrapped around a metal conduit used as permanent wiring to energize a table grinder exposing employees to electrical shock hazards.

29 CFR  1910.305(b)(1): Holes in electrical equipment (boxes, Panels, etc) were not effectively closed (missing breakers, etc.)   On or about September 21, 2024, and times prior thereto, electrical disconnect box of the south side of the machine shop missing a large knock-out plug exposing employees to electrical shock hazards.
