29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by a guardrail system, safety net system, or personal fall arrest system, or an alternative fall protection measure that meets a provision described in paragraph (b) of this section:  On or about February 28, 2024, at 12973 SW 266th Terrace, Homestead, Florida, employees were exposed to 19-foot fall hazards while installing plywood on a new residential roof construction without a means of fall protection.

29 CFR  1926.1053(b)(1): Where portable ladders were used to access an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet above the upper landing surface being accessed; the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:  On or about February 28, 2024, at 12973 SW 266thTerrace, Homestead, Florida, employees were exposed to 11-foot fall hazards while accessing a roof tp install plywood using an extension ladder that was neither extended 3 feet over the landing surface nor was secured at its top to a rigid support.
