29 CFR 1904.41(a)(1)(i): If your establishment had 20-249 employees at any time during the previous calendar year, and your establishment is classified in an industry listed in appendix A to subpart E of this part, then you must electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee. You must submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form. (a) DSI Logistics, LLC: On or about March 2, 2024, the employer for calendar year 2023 did not electronically submit information from the OSHA Form 300A or equivalent by March 2, 2024. 29 CFR 1910.178(q)(7): Industrial trucks were being placed in services after the examination showed condition(s) adversely affecting the safety of the vehicle: a) Warehouse: On or about February 28, 2024, and times prior, the employer exposed employees to potential struck-by hazards, in that two forklifts equipped with clamp attachments were not removed from service or repaired prior to use. Forklift LT1516 had two worn and slit back tires. b) Warehouse: On or about February 28, 2024, and times prior, the employer exposed employees to potential struck-by hazards, in that two forklifts equipped with clamp attachments were not removed from service or repaired prior to use. Forklift LT4721 did not have an operational reverse light.
