Forklift LT1516 had two worn and slit back tires.  b)  Warehouse: On or about February 28, 2024, and times prior, the employer exposed employees to potential struck-by hazards, in that two forklifts equipped with clamp attachments were not removed from service or repaired prior to use.  Forklift LT4721 did not have an operational reverse light.
