29 CFR  1904.41(a)(1)(i): If your establishment had 20-249 employees at any time during the previous calendar year, and your establishment is classified in an industry listed in appendix A to subpart E of this part, then you must electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee. You must submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form.   (a)  DSI Logistics, LLC:   On or about March 2, 2024, the employer for calendar year 2023 did not electronically submit information from the OSHA Form 300A or equivalent by March 2, 2024. The establishment employed 125 employees and was classified in the North American Industrial Classification System as 493110 during calendar year 2023.

29 CFR  1910.178(q)(7): Industrial trucks were being placed in services after the examination showed condition(s) adversely affecting the safety of the vehicle:   a)  Warehouse: On or about February 28, 2024, and times prior, the employer exposed employees to potential struck-by hazards, in that two forklifts equipped with clamp attachments were not removed from service or repaired prior to use.  Forklift LT1516 had two worn and slit back tires.  b)  Warehouse: On or about February 28, 2024, and times prior, the employer exposed employees to potential struck-by hazards, in that two forklifts equipped with clamp attachments were not removed from service or repaired prior to use.  Forklift LT4721 did not have an operational reverse light.
