29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) 1249 Trexlertown Rd., Trexlertown, PA 18087: On or about March 4, 2024, employees performing roofing/flashing work, approximately 20 feet above grade and no means of fall protection was being utilized.

29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) 1249 Trexlertown Rd., Trexlertown, PA 18087 : On or about March 4, 2024, the aluminum extension ladder utilized to access the roof was not erected to allow the ladder side rails to extend 3 feet above the landing area.
