29 CFR  1926.405(g)(2)(iv):Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  At the jobsite located at 17763 Shady Bend, Gardner, Kansas, employees engaged in residential framing activities were exposed to electrical shock hazards in that the extension cords, providing 220 VAC power to Roland air compressor, protective layer was pulled away from the plug end of the cord, exposing the electrodes.

29 CFR  1926.416(e)(1):Worn or frayed electric cords or cables were used:  At the jobsite located at  17763 Shady Bend, Gardner, Kansas, employees engaged in residential framing activities were exposed to electrical hazards in that the extension cords providing 220 VAC power were significantly damaged.  The insulation had pulled away and frayed, exposing the conductors.
