29 CFR  1926.20(b)(1):The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  On or about March 4, 2024 the employer Woodson's Exterior Remodeling, LLC at jobsite located at 9700 Dutch Rd. Whitehouse OH 43571, did not have a plan or a program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls and struck by hazards during job site operations, decreasing the probability of employee injury or illness.

Whitehouse OH 43571, did not have a fall protection training program in place to enable each employee to recognize the hazards of falling, and procedures to minimize these hazards. Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury or illness.

Whitehouse OH 43571, did not ensure that employees were protected from head injury hazards by protective hardhats while working directly below employees who were using hand tools and installing roofing underlayment and shingle materials, which was an overhead struck-by hazard.

Whitehouse OH 43571, did not ensure employees were protected from falls while conducting residential roofing construction activities on the second (roof) level of new build single family home, while exposed to a fall of between approximately 12 feet and 18 feet or higher to the ground below. The roof was 8:12 pitch.
