29 CFR 1910.36(d)(1):Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge: Location: Back room, walk-in freezer. On or about 3/6/24, employees were exposed to hypothermia and other cold-related injury due to a walk-in freezer door that was outfitted with a padlock. The employer did not ensure that the freezer door was outfitted with hardware that allowed employees inside the freezer to be able to exit despite the locked/unlocked status of the door.

On or about 3/6/24, the employer did not establish a written hazard communication program that included the required elements of the program. By not establishing a program, the employer could not ensure that all elements were met to provide employee protection from chemicals including, but not limited to Clean Quick.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: Location: Back room, wash sink. On or about 3/6/24, the employer did not train all employees on the chemicals that they work with in the workplace, including, but not limited to Clean Quick. By not training employees on chemicals, employees may not be able to protect themselves from exposure, or what to do if exposure occurs.
