29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could result in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions and were not conducted by the competent person prior to the start of work and as needed throughout the shift, including after every rainstorm or other hazard increasing occurrence:    a) Job site, 958 Ohio River Boulevard, Avalon, PA: On or about March 7, 2024, during water line installation activities, the employer did not ensure that a competent person evaluated the excavation and on-going operations associated with it, for safety and health concerns. Deficiencies noted during the inspection included, but were not limited to, failing to ensure that previously removed items were placed at least 2 feet from the edge of the excavation, not ensuring that a safe way in/out of the excavation was provided and not having appropriate shoring in use in areas where the top layer of asphalt was significantly undercut.
