29 CFR 1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about March 7, 2024, employees were exposed to injuries from rotating parts and flying chips and the employer did not ensure machines were appropriately guarded:  a.  The Mazak V515/40 CNC was being operated with the doors open and the interlocks were not operational.  b. The Mazak V515/40 CNC was missing the guards used to cover the automatic tooling chain.  c. The Clausing EVS vertical mill, Tridan #001189 in the South Building was used for metal parts fabrication and chip guards were not used.  d. The Clausing Kondia vertical mill, Tridan #001024 in the South Building was used for metal parts fabrication and chip guards were not used.  e. The Clausing Kondia vertical mill, Tridan #001023 in the South Building was used for metal parts fabrication and chip guards were not used.  f. The Clausing Kondia vertical mill, Serial Number S-424, Tridan #001022 in the South Building was used for metal parts fabrication and chip guards were not used.  g. The Mazak Quick Turn 250 HP in the South Building and was missing the guard covering the outside bar spindle.
