29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury:  MDC Capitol Avenue Area Water Main Replacement project, 193 Sisson Avenue, Hartford, Connecticut:  On, or about, March 8, 2024, an employee working as a flagger was struck by a moving work truck while performing his work duties.  The employer did not ensure employees deemed as flaggers were trained in accordance with Part VI, Section 6D.01 Qualifications for flaggers of the Manual on Uniform Traffic Control Devices (MUTCD), 11th Edition.
