(b)Worthington Tractor Parts, Inc., dba Worthington Ag Parts at 5300 North Annika Avenue, Sioux Falls, SD 57107: On or about and at times prior to March 11, 2024, the employer did not ensure that relocatable power taps were properly utilized in that a relocatable power tap located behind the packing foam station was used as a substitute for permanent wiring. This condition exposed employees to an increased risk of fire hazard, blocked egress, the potential for trip/fall hazards, and the inability to access first aid equipment in the event of a medical emergency.
