At the commercial facility located at 700 E 16th Avenue in North Kansas City, Missouri 64116: On or about March 12, 2024, employees working at the facility were exposed to respiratory hazards including asthma, trouble breathing, claustrophobia, and other medical conditions when the employer provided half-face tight-fitting elastomeric respirators before ensuring that a medical evaluation was performed to determine each employee's ability to wear a respirator.

29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck.  At the commercial facility located at 700 E 16th Avenue in North Kansas City, Missouri 64116: On or about March 12, 2024, employees working at the facility were exposed to struck by hazards in the form of improper use, seatbelts, driving speed, and unsecured loads.  The employer exposed the employee to these hazards by allowing them to operate a powered industrial truck without first ensuring that the employee received initial training for operating the powered industrial truck.
