29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. On or about March 12, 2024, at an establishment located at 1111 Air Park Road in Rhinelander, Wisconsin, employees of Hypro, Inc. were not provided machine guarding to protect operators and other employees in the machine areas from hazards, such but not limited to: a) The employer did not protect employees from an ingoing nip point on a parts conveyor in the east building on RLC3. b) The employer did not protect employees from an ingoing nip point on a parts conveyor in the east building on RLC4. c) The employer did not protect employees from an exposed auger in the south building on R37. d) The employer did not protect employees from a rotating machine part in the north building on machine 295. e) The employer did not protect employees from rotating machine parts in the north building on the hobbing machine 412. f) The employer did not protect employees from rotating machine parts in the east building on the machine 288 grinder.

29 CFR 1910.303(b)(2): Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling. At an establishment located at 1111 Air Park Road in Rhinelander, Wisconsin, employees of Hypro, Inc. were exposed to electrical and fire hazards utilizing listed equipment not in accordance with the instructions for a Relocatable Power Tap (RPT) that was not designed to be installed in an industrial setting in the following settings: a) On or about March 12, 2024, employees in the south building were exposed to electrical and shock hazards near RO1 Vertical Milling Center. b) On or about March 12, 2024, employees in the south building were exposed to electrical and shock hazards near Machine 276 Horizontal Lathe. c) On or about March 12, 2024, employees in the south building were exposed to electrical and shock hazards near Machine R51 Horizontal Lathe. d) On or about March 12, 2024, employees in the north building were exposed to electrical and shock hazards near Machine 303 Horizontal Lathe.

On or about March 12, 2024, at an establishment located at 1111 Air Park Road in Rhinelander, Wisconsin, employees of Hypro, Inc. were exposed to electrical shock and burn hazards when the employer did not protect employees from electrical hazards in the South Building on Machine R37 Horizontal Milling Machine where conductors entered a junction box on a motor with the fitting pulled out of the junction box, exposing the wires to abrasion.
