On or about March 12, 2024, at NE corner of Orange Blossom Trail and Sand Lake Rd., employees were exposed to crush-by hazards while cutting a 36" opening on a 60" concrete storm drain pipe in a 10 foot deep trench with near vertical walls in type C soil, in that cave in protection was not used.
