29 CFR 1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).  a. On or about March 12, 2024, at NE corner of Orange Blossom Trail and Sand Lake Rd., employees were exposed to crush-by hazards while cutting a 36" opening on a 60" concrete storm drain pipe in a 10 foot deep trench with near vertical walls  in type C soil, in that cave in protection was not used.

On or about March 12, 2024, at NE corner of Orange Blossom Trail and Sand Lake rd., employees cutting 36" opening on a 60" concrete storm drain pipe were using  an "A" frame ladder on its closed position to access and exit a 10 foot deep trench, exposing employees to fall hazards.

On or about March 12, 2024, at NE corner of Orange Blossom Trail and Sand Lake rd, employees cutting 36" opening on a 60" concrete storm drain pipe were using a closed 10 foot A frame ladder to access and exit an approximate 10 foot deep trench, exposing employees to fall hazards.
