1910.132(d)(1)  The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment:  a) On or about 3/5/2024 at worksite: the employer allowed the operation of the Grizzly Industrial Inc. 12" Grinder model G7297, while using the assigned work gloves, however did not retain the specific training on not wearing gloves while operating rotating tools (grinders).

29 CFR  1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  a) On or about 3/5/2024 at  the truss shop,  Employer failed to report to OSHA within 24 hours a work-related in-patient hospitalization of an employee. The employer notified OSHA on or about 3/12/2024 of the in-patient hospitalization.
