29 CFR  1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  a) Residential home/Roof, located at 4627 Eberline CT, Stone Mountain, GA 30083 : On or about 3/7/2024, and at times prior, the employer exposed employees to struck-by hazards in that, the employer failed to ensure that eye protective equipment was provided to employees using pneumatic nail guns at 120 PSI while engaged in roofing activities.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)Residential home/Roof located at 4627 Eberline CT., Stone Mountain, GA 30083 : On or about 3/7/2024, and at times prior, the employer exposed employees to fall hazards in that, the employer failed to ensure that fall protection system was provided to employees engaged in roofing activities of approximately 16-feet and 11-inches above the ground level.   b) Residential home/Roof located at 4627 Eberline CT., Stone Mountain, GA 30083 : On or about 3/7/2024, and at times prior, the employer exposed employees to fall hazards in that, the employer failed to ensure that fall protection system was provided to employees engaged in roofing activities of approximately 8-feet and 3-inches above the ground level.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Residential home/Roof located at 4627 Eberline CT., Stone Mountain, GA 30083: On or about 3/7/2024, the employer exposed employees to fall hazards, in that the employer failed to ensure that a stepladder that used by employees to access the upper level was extended 3-feet above the upper landing surface which was approximately 16-feet and 11-inches above the ground level.  b) Residential home/Roof located at 4627 Eberline CT., Stone Mountain, GA 30083: On or about 3/7/2024, the employer exposed employees to fall hazards, in that the employer failed to ensure that a stepladder that used by employees to access the upper level was extended 3-feet above the upper landing surface which was approximately approximately 8-feet and 3-inches above the ground level.
