29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use:   a) Jobsite located at or near 348 Bridge Street, Phoenixville, PA- On or about March 14, 2024, the employer did not ensure the platform used to elevate an employee from the forks of the rough-terrain forklift was specifically designed by the manufacturer for such use, thereby exposing employees to fall hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b) of this section.  a) Jobsite located at or near 348 Bridge Street, Phoenixville, PA- On or about March 14, 2024, the employer did not ensure an employee engaged in residential construction activities 6 feet or more above lower levels was protected by guardrail systems, safety net system, or personal fall arrest system. Employees were framing the building approximately 27 to 36 feet above ground level without the use of a fall protection system.  Solutions Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1666170, citation number 1, item number 1 and was affirmed as a final order on November 21, 2023, with respect to a workplace located at 829 N. 19th St. Philadelphia, PA.  Solutions Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1647398, citation number 2, item number 1 and was affirmed as a final order on August 22, 2023, with respect to a workplace located at 2002 Brown St, Philadelphia, PA   Solutions Construction, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1553242, citation number 1, item number 1-a, and was affirmed as a final order on March 8, 2022, with respect to a workplace located at 2500 E Tioga St Philadelphia, PA 19134.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee exposed to fall hazards to enable them to recognize the hazards of falling and the procedures to follow in order to minimize them:  a) Jobsite located at or near 348 Bridge Street, Phoenixville, PA- On or about March 14, 2024, the employer did not provide fall protection training to employees who were exposed to fall hazards.  Employees were performing framing activities from heights greater than 6 feet without being trained on fall protection, thereby exposing employees to fall hazards.
