29 CFR 1926.100(a): Head Protection - Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.  168 Belden Street: On or about March 24, 2024, during a roofing project with employees dropping shingles and other debris from the roof to the ground, employees working on the ground collecting the debris for disposal were not wearing head protection/safety helmets/hard hats.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  168 Belden Street: On or about March 24, 2024, employees were identified performing residential construction activities.  Two portable extension ladders were being used by Employees to access upper landing surfaces were positioned without the side rails being extended 3-feet past the upper landing surface.
