29 CFR 1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects. For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall. On or about March 14, 2024, on the south side of the jobsite, an employee was performing masonry work from the top level of the scaffold that did not have toeboards installed, or another means of falling object protection installed, while another employee worked below.

29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilivered end. On or about March 14, 2024, on the south side of the jobsite, employees were performing masonry work activities from a scaffold with planks that extended more than 18 inches over the support.
