29 CFR  1904.29(b)(3): The employer did not enter each recordable injury or illness on the OSHA 300 Log and 301 Incident Report within seven (7) calendar days of receiving information that a recordable injury or illness had occurred:  a) On or about December 8, 2023, the employer did not enter on the OSHA Form 300, the log of Occupational Injury and Illness, a recordable fingertip crushing injury, that occurred on December 1, 2023, and resulted in days away from work.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR  1910.178(l)(6): The employer did not certify that each operator had been trained and evaluated as required by paragraph (l).  The certification did not include the name of the operator, the date of training, the date of the evaluation, and the identity of the person(s) performing the training and evaluation.   a) Production Area - On March 15, 2024, the employer did not ensure that employee(s) who operate the forklifts in the facility, had their training and evaluation certified including the name of the operator, the date of the evaluation, and the identity of the person performing the evaluation.     In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
