29 CFR 1910.178(m)(3): Unauthorized personnel were permitted to ride on industrial truck(s):   On or about March 15, 2024, employees were allowed to ride on forklifts in the following instances:  a)  employees were allowed to be elevated on the forks of the forklift, such as but not limited to, replacing a fuse on a crane while standing on a pallet supported by a forklift exposing the employees to fall hazards, and  b) employees were allowed to ride on the forklift with the operator during training on the forklift and other activities exposing employees to falling hazards and struck-by hazards.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) was not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket). Cord and plug connected equipment and flexible cord sets (extension cords) which remain connected once they are put in place and are not exposed to damage need not be visually inspected until they are relocated.  On or about March 19, 2024, and at times prior thereto, the employer did not ensure each portable cord and plug connected equipment was visually inspected before use on any shift for external defects.
