29 CFR  1910.28(b)(3)(v): The employer did not ensure that each employee is protected from falling through a hatchway:  a.  On or about March 20, 2024, employees required to access the walking and working surface in the attic were exposed to falls of approximately ninety-four and one-half inches (94.5").  The attic access hatchway was not guarded to prevent employees from falling through the open hatchway while working and/or descending from the attic.
