Employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest systems: On or about March 20, 2024, employees of Javier Eric Cruz, dba: Ambition Construction were performing roofing work at 744 Ballantrae Drive in Wentzville, Missouri. The employer did not ensure that fall protection systems were utilized by employees. The employees were installing shingles approximately 13 feet above the lower level. The employees were exposed to fall hazards to the ground below.

29 CFR 1926.1053(b)(15): Ladder(s) were not inspected by a competent person for visible defects on a periodic basis and/or after an occurrence affecting their safe use: On or about March 20, 2024, employees of Javier Eric Cruz, dba: Ambition Construction were performing roofing work at 744 Ballantrae Drive in Wentzville, Missouri. A Louisville aluminum extension ladder with visibly damaged rungs, siderail and locking mechanism was placed on the residence for access and egress to the roof. The ladder had not been inspected by a competent person exposing employees to fall hazards.
