29 CFR 1926.501(b)(10):  Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, warning line system and personal fall arrest system, or warning line system and safety monitoring system;  On or about March 20, 2024, the employer failed to ensure that each employee engaged in roofing activities at Auburn City Hall - 1101 J Street - Auburn, Nebraska 68305,  at heights exceeding 10 feet above lower levels were protected from fall hazards by fall arrest systems, guardrails, or safety monitoring systems. Four employees,  engaged in sheet metal removal and installation, were not provided any form of fall protection while working on the roof of the commercial building.

29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access, nor was the ladder secured at it's top to a rigid support and a grasping device, such as a grabrail, provided to assist the employee in mounting and dismounting the the ladder:  On or about March 20, 2024, the employer failed to ensure that each employee accessing the roof area at Auburn City Hall - 1101 J Street - Auburn, Nebraska 68305, was protected from fall hazards by ensuring that the unsecured ladder was extended at least three feet above the upper landing surface.

29 CFR  1926.1060(a)(1)(iii): The employer did not ensure that each employee using ladders and stairways had been trained by a competent person in the proper construction, use, placement, and care in handling of all stairways and ladders:   On or about March 20, 2024, the employer failed to ensure that each employee accessing the roof area at Auburn City Hall - 1101 J Street - Auburn, Nebraska 68305, was properly trained by a competent person in the use and placement of the jobsite ladder.  The unsecured ladder was not extended at least three feet above the upper landing surface.
