29 CFR 1910.212(a)(3)(ii):The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. (a) On or about March 21, 2024, the safety interlocks were not properly working on the Hurco VMX 50 CNC machine (Model No. VM 50 and Serial No. M542-01010041AFC). This condition allowed employees to operate the machine with the doors open, which exposed employees to an entanglement hazard. (b) On or about March 21, 2024, the safety interlocks were not properly working on the Hurco VMX 42 CNC machine (Model No. VM 42 and Serial No. M442-06002016DHA). This condition allowed employees to operate the machine with the doors open, which exposed employees to an entanglement hazard. (c) On or about March 21, 2024, the safety interlocks were not properly working on the Haas VF3 CNC machine (Model No. VF3 and Serial No. 17916). This condition allowed employees to operate the machine with the doors open, which exposed employees to an entanglement hazard.

(a) On or about March 21, 2024, the Delta Table Saw, located in the Wood Pattern Shop was not properly guard, in-that the saw blade did not have an adjustable hood guard installed. This condition allowed employees to operate the saw with the saw blade being exposed, exposing employees to an amputation hazard.
