1910.147(c)(6)(ii):  The employer shall certify that the periodic inspections have been performed. The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection.  On or about March 17, 2024, at a facility located at 718 High Street in New London, Wisconsin, an employee was exposed to machine hazards associated with moving parts while engaged in a maintenance task.    The Company did not fully document its certification of its periodic inspection of the procedures for deenergizing ink pump #8 on the flexo-press #8 machine.

29 CFR 1910.219(c)(3):   Guarding vertical and inclined shafting. Vertical and inclined shafting seven (7) feet or less from floor or working platform, excepting maintenance runways, shall be enclosed with a stationary casing in accordance with requirements of paragraphs (m) and (o) of this section.  On or about March 17, 2024, at a facility located at 718 High Street in New London, Wisconsin, employees were not provided machine guarding to protect operators and other employees in the flexo press department near flexo-press #8 on the white ink barrel spinning vertical shafting.
