29 CFR  1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions.   (a) Centennial Custom Flatwork Co., 1525 - 1555 Sherman St. (Alley), Denver, CO 80203: On or before March 21, 2024, the employer did not ensure that three employees were protected from hazards associated with spoils piles, suspended loads and lateral pressure applied to trench walls in that no inspections were conducted by the competent person as required. This exposed employees to struck by and caught in hazards.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c):  (a) Centennial Custom Flatwork Company, 1525 - 1555 Sherman St. (Alley), Denver, CO 80203: The employer did not ensure effective protection against cave ins, as demonstrated on or before March 21, 2024, and times prior, when employees were exposed to cave in hazards while removing material and debris from the trench where an underground fuel tank had been removed when the excavation was greater than 5 feet deep.

29 CFR 1926.1053(b)(1): A portable ladder was used for access to an upper landing, and the ladder side rails did not extend at least 3 feet above the upper landing, or was not secured:  (a)	Centennial Custom Flatwork Co., 1525-1555 Sherman Street (Alley), Denver, CO, 80203:  On or before March 21, 2024, the employer did not ensure that employees were protected from fall hazards in that extension ladder did not extend 3 feet above the upper landing edge of the excavation.  Employees accessed the excavation using extension ladders to an upper concrete surface edge.  This condition exposed employees to fall hazards of approximately 6 to 10 feet into the excavation below.   (b)	Centennial Custom Flatwork Co., 1525-1555 Sherman Street (Alley), Denver, CO, 80203:  On or before March 21, 2024, the employer did not ensure that employees were protected from slip and/or fall hazards when using an extension ladder to access an excavation in that the ladder extension was positioned such that deflection under a load, by itself, would cause the ladder to slip off its support.  This condition exposed employees to fall hazards of approximately 6 to 10 feet into the excavation below.
