29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).  On or about March 21, 2024, at the open trench on the jobsite; employees utilized one trench box and manhole for a trench that was greater than five feet in depth without an adequate protective system from cave-ins while performing underground sewer pipe installation for an ongoing development of a new neighborhood.

29 CFR 1926.652(g)(1)(ii): Shields shall be installed in a manner to restrict lateral or other hazardous movement of the shield in the event of the application of sudden lateral loads.  On or about March 21, 2024, at the open trench on the jobsite; employees utilized one trench box and manhole guard for a trench that was greater than five feet in depth without stabilizing the guards to prevent movement while performing underground sewer pipe installation for an ongoing development of a new neighborhood.
