29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  On February 23, 2024, an employee fell from a ladder resulting in an in-patient hospitalization of the employee. The employer did not report to OSHA within 24-hours the in-patient hospitalization of the employee. The event occurred at approximately 6:30PM on February 23, 2024, but was not reported to the local office of the Occupational Safety and Health Administration until February 26, 2024, at approximately 11:57AM.
