29 CFR  1926.100(a):Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  On or about March 20, 2024, and at times prior thereto, employees were installing shingles on the roof of a 2-story home under construction above another employee working on the ground without a hard hat, exposing the employee to the hazard of being struck-by falling objects.

29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about March 23, 2021, and at times prior thereto, employees were using pneumatic roofing nailers to install shingles on a residential roof under construction without the use of eye protection, exposing employees to eye hazard.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  On or about March 20, 2024, and at times prior thereto, employees were descending the roof of a residential home by means of a ladder that did not extend at least 3 feet above the upper landing surface, exposing employees to a fall hazard.
