Needham, MA  On March 25, 2024, an employee  exposed to overhead struck-by hazards from materials while working at ground level while using a materials hoist to raise materials to the roof and also where other employees, tools and equipment was on the roof where protective helmets were not utilized

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  Location: 234 Valley Rd, Needham, MA  On March 25, 2024, employees were exposed to falls from heights up to 25 feet to the ground while accessing an upper landing surface where an extension ladder was used to access a residential roof and the ladder side rails did not extend at least 3 feet above the edge of the roof.
