29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  6 Hastings Ave., Millbury, MA: On or about March 27, 2024, an employee was observed working on a residential structure engaged in roofing activities with unprotected sides and edges greater than 10 feet above the ground below and was not protected by either guard rails, safety net systems or personal fall arrest systems.

1926.503(b)(1)  The employer did not verify fall protection training by preparing a written certification record:  6 Hastings Ave., Millbury: On or about March 27, 2024, the employer did not assure that employees, while working at a fall height of approximately 10 feet, were trained in both the nature of fall hazards in the work area and the correct procedures for erecting, maintaining, disassembling, and inspecting the fall protection systems that were used.
